The following required forms MUST be completed and received via mail, fax or email
14 days prior to the first day of camp:
Release Form
Physical Examination Form 
Activity Decline Form

If you register for High School 1, High School 2 or Middle School Challenge,
the Rafting Release Form MUST also be submitted in addition to the three forms above. Thank you.
Lastly, if your child does not receive immunizations, please complete the Statement of Exemption to Immunization and submit it with the other required forms above.

For those of you attending Young at Heart adult camp, click on the following link for the registration form:

Please remit a $5.00 processing fee for each mail-in registration form.
 
Download and print the applicable forms below.
Remember to have all forms and either a $50.00 deposit or payment in full sent to Camp Como at least 14 days prior to the first day of camp.

Youth Registration Form
Junior Faculty Registration Form (18 years of age or younger)
Adult Faculty Registration Form (Youth Ministers, Sponsors, etc.)

Don't forget the Release, Physical Examination and Activity Decline Forms which can be found above. Also, remember the Rafting Release Form for those who are attending High School 1, High School 2 or Middle School Challenge camps as well as our helpful General Registration Information page which can also be found above.  Please note:  Adult Faculty MUST submit the Noah's Ark Rafting Release if applicable.  Thank you.
